ASSOCIATION BETWEEN SLEEP DISORDERED BREATHING AND GLUCOSE METABOLISM IN EARLY PREGNANCY

چکیده

TOPIC: Sleep Disorders TYPE: Original Investigations PURPOSE: disordered breathing (SDB) is common in pregnancy. A relationship has been described late pregnancy between SDB and abnormal glucose metabolism, gestational diabetes mellitus (GDM). However, it remains unclear whether this starts early or develops following exposure to pregnancy. The goal of the current study examine association its severity with metabolism METHODS: Eligible participants were women singleton pregnancies who had risk factors for obstructive sleep apnea such as body mass index (BMI) ≥ 27 kg/m2 habitual snoring. Exclusion criteria included chronic steroid therapy, history pregestational diabetes, use continuous positive airway pressure.Participants underwent in-home level III testing (HSAT) homeostatic model assessment (HOMA) based on fasting C-peptide levels We derived measures (respiratory-event (REI) oxygen saturation (ODI)) parameters (insulin resistance (HOMA-IR) ꞵ-cell function (HOMA-%B)). performed linear regression analysis variance parameters. Models adjusted age at testing, maternal age, BMI, ethnicity, race. RESULTS: 61 192 (32%) diagnosed OSA REI values 5 events/hour. HSAT HOMA a median (interquartile range) 11 (3) 15 (4) weeks, respectively. Women higher insulin compared those without. REI, value, was significantly associated HOMA-IR (B=0.18, p=0.041) after adjustment covariates, while diagnosis (REI events/hour) not adjusting BMI (p>0.05).ODI prior covariates (B=0.22, p=0.002) but dropped below significance (p=0.24). There no significant associations ODI HOMA-%B (p>0.34). CONCLUSIONS: Our results support that increasing severity, measured by increased values, This because relationships are largely unstudied. CLINICAL IMPLICATIONS: data suggest may enter GDM, instead developing over course could imply screening treatment should occur earlier Further studies needed investigate if improve metabolism. Sleep disordered breathing in pregnancy

KEY POINTS Sleep disordered breathing (SDB) is common and the severity increases as pregnancy progresses.Frequent snoring, older age and high pre-pregnancy body mass index (>25 kg⋅m(-2)) could be reliable indicators for SDB in early pregnancy.SDB screening tools, including questionnaires, used in the nonpregnant population have poor predictive ability in pregnancy.Accumulating evidence suggests...

Sleep-disordered breathing during pregnancy.

Pregnancy is associated with many physiologic and hormonal changes along with changes in sleep architecture, placing pregnant women at risk for the development of sleep-disordered breathing or worsening of preexisting sleep apnea. Snoring, the most common symptom of sleep-disordered breathing, is markedly increased during pregnancy. The exact prevalence of obstructive sleep apnea in pregnant wo...

Association between metabolic syndrome and sleep-disordered breathing in adolescents.

RATIONALE Metabolic syndrome (MetS) affects 4 to 10% of adolescents. Risk factors include overweight, male sex, and Hispanic ethnicity. Although sleep-disordered breathing (SDB) has been implicated as a risk factor for MetS in adults, its association with SDB in adolescents is unknown. OBJECTIVES To define the association of SDB with MetS in adolescents. METHODS Standardized measurements of...

OCCASIONAL REVIEW Sleep disordered breathing and pregnancy

Many changes in the respiratory system occur during pregnancy, particularly during the third trimester, which can alter respiratory function during sleep, increasing the incidence and severity of sleep disordered breathing. These changes include increased ventilatory drive and metabolic rate, reduced functional residual capacity and residual volume, increased alveolar-arterial oxygen gradients,...

Outcomes of Sleep Disordered Breathing in Pregnancy

Sleep disordered breathing has been linked to adverse cardiovascular outcomes in the general population. In pregnancy, sleep disordered breathing has also been linked to pathologic disorders that have been associated with long term cardiovascular and metabolic outcomes such as gestational hypertensive disorders and gestational diabetes mellitus. Endothelial dysfunction, sympathetic stimulation ...
